29 CFR 1910.213(r)(4): Suitable guard(s) and exhaust hood(s) were not provided to reduce to a minimum the hazard due to the point of operation of the such woodworking machine(s): Facility: The point of operation guard of the DeWalt compound Miter saw, DWS779 & SN 109826, was not functioning properly and kept in an up position. On or about May 8, 2018, an employee's finger came in contact with the unguarded saw blade while operating the above saw, and the employee sustained severe laceration requiring hospitalization.
