29 CFR 1915.15(e): After a competent person conducted a visual inspection and tests and determined a space to be safe for an employee to enter, he or she did not continue to test and visually inspect spaces as often as necessary to ensure that the required atmospheric conditions within the tested space were maintained. (a) Date of Violation: On or about 08-20-2018 and times prior thereto, located at Norfolk Naval Shipyard, Portsmouth, VA 23709, onboard and beneath dry-docked submarine USS Wyoming (SSBN 742): Employee(s) were exposed to unknown atmospheric conditions inside the TEP tube. Confined space was initially tested by a competent person and was not tested again or maintained after local exhaust ventilation was applied to the space. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
