29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: A) Materion Brush, Inc., located at 14710 West Portage River South Road, Elmore, OH 43416: On or about August 15, 2018, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the Eitel Stroke Controlled Straightening Press # 1 (Type RP 60, Machine # 25374) prior to set-up activities. Employees conducted set-up activities such as, but not limited to, changing ram heads at the Eitel Press # 1 and were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
