29 CFR 1904.8(a): All work-related needlestick injuries and cuts from sharp objects that were contaminated with another person's blood or other potentially infectious material were not recorded on the OSHA 300 Log as an injury. a) 9300 Dewitt Loop, Ft Belvoir, VA 22060: On or about August 15, 2018, and times prior thereto, sharps injury cases were not were not marked in column M(1) or other for cases that were classified on the form in Columns G, H, I, or J. The cases were not marked as an injury or an illness for 2013, 2014, 2015, 2016, 2017, and 2018 forms, and the error was carried forward to the 300A for each year. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
