29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment: a) Field 21 - On or about August 9, 2018 and at times prior thereto, employees who operated HAK tractors were not trained to shut them down prior to unclogging the equipment. On July 31, 2018 an employee was hospitalized following an incident where a HAK tractor was left running when the employee was removing weeds/grass from the implements on the tractor.
