29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a) Spray Dryer Room No. 2 - On or about September 13, 2018, employees were required to enter into the Spray Dryer Chamber to perform cleaning. The concrete walkway floor leading up to the Spray Dryer Chamber's manway entry door had a floor hole measuring 5.5 inches in its least dimension, which increased up to 12 inches around the diameter of the Spray Dryer, and was 47 inches above the lower level. The employer failed to ensure each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.
