29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 125 Gloucester Avenue, Gloucester, MA, Building 1, VTL section VTL and CNC machines were not provided guarding to protect the operator and other employees in the machine area from hazards created by rotating parts of the machines. This includes, but is not limited to the following instances: a) On or about August 1, 2018, employees were exposed to hazards on the Gray VTL Shop ID# 10038. The rotating table was not guarded by a method of guarding that prevented employees from contacting the table. b) On or about August 25, 2018, employees were exposed to hazards on the Summit VTL Shop ID# 13104. The rotating table was not guarded by a method of guarding that prevented employees from contacting the table. 125 Gloucester Avenue, Gloucester, MA, Building 2, main shop floor c) On or about August 27, 2018, employees were exposed to hazards on the Okuma OSP700M CNC Shop ID# 13233. The CNC door interlock was found to be defeated, in that the key was removed from the sliding door and placed in the interlock, allowing the machine to operate with the door in the open position, exposing employees to struck by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
