Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about the 11 day period of July 10 - 31, 2018, there were no frequent and regular inspections being performed by a competent person at a commercial demolition site in that: (a) employees were performing interior demolition work, on three different days, at fall heights greater than 24 feet and 31 feet above the ground below without a fall protection system (b) employees were using exterior fixed stairs, for gaining access to elevated work areas for the purpose of demolition, that had most of the top stair landing and guardrails removed (c) employees were using exterior fixed stairs, for gaining access to elevated work areas for the purpose of demolition, that had the remainder of the top stair landing set at a depth of 11 inches in the direction of travel and without guardrails (d) employees were using exterior fixed stairs, for gaining access to elevated work areas for the purpose of demolition, that had 1 uncovered floor hole measuring 5 feet long by 9 inches wide at the 2nd story stair landing (e) employees were using exterior fixed stairs, for gaining access to elevated work areas for the purpose of demolition, that had 2 damaged stair treads with a variation in depth greater than 1/4 inch (f) employees were using exterior fixed stairs, for gaining access to elevated work areas for the purpose of demolition, that required employees to step across an open space of at least 1 foot to transition from the unprotected top stair landing to the structure at a fall height of greater than 24 feet (g) employees were working at ground level near the base of a partially demolished building that had an uncovered floor hole, measuring 7 feet by 3 feet by 2 feet deep, located where the total floor is so large a worker may lose track of the location of the floor hole and either trip or step into it; and (h) a Supervisor was operating an aerial lift with a platform gate that was damaged and would not latch shut.
