29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: The employer is failing to ensure that employees are fit tested at least annually. The most recent occurrence was documented during the inspection conducted on July 30, 2018 on the jobsite located at 10004 NW 112th St, Malcolm, NE 68402. The employer has employees using tight fitting respirators when applying spray foam insulation. This process is conducted approximately three to five times per week. Employees are given an initial fit test; however, the employer is not ensuring employees are fit tested at least annually thereafter.
