29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by a pinch-point: At 3125 Boschertown Road, St. Charles, MO, specifically inside of the Galvanized Department, the clamp die arm belonging to the UTE CNC Tube Bender Machine, m/n M80CNCBENDER, s/n 959, was unguarded. Two fingers on the operator's right hand were amputated on or about 07/26/18.
