29 CFR 1910.176(b): Secure storage. Storage of material created a hazard. Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse: On and before 07/24/2018, at Eagle Nonwovens Inc., located at 10301 Lake Bluff Drive, St Louis, the employer exposed employees to struck-by hazards when it failed to stack bales in such a way that they were secure against sliding or collapse. The most recent instance of this occurred on 07/24/2018 when the employer stacked four 700 pound bales on top of one another against a wall at line #4. The bales were not stacked, blocked, interlocked and limited in height so that they were secure and stable. As a result, the bale on the top stack of four slid from the top, striking and killing an employee.
