29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience. (a) On or about 07/10/18, during a roofing project at the St. Pius X School, located at 3000 Chili Avenue, in Rochester, New York, the employer did not provide or ensure an employee assigned to operate an ATV ( Honda 2016 TRX250TE), was qualified or trained to safety operate the equipment in the following areas: 1. Not instructed to read "Owner's Manual". 2. Never instructed/trained on performing a pre-ride inspection. 3. Never instructed/trained about testing the throttle. 4. Never instructed/trained on the throttle inspection. 5. Never instructed/trained on "starting and stopping" procedures. 6. Never instructed/trained about the "emergency stop" process.
