Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Courtyard Area- On or about July 5, 2018, the employer and the outside servicing personnel of Catalytic Products International, Inc. as well as Advance Technology Services Inc., did not inform one another of their lockout tagout procedures. All three companies had procedures, but none of the three were utilized and it contributed to confusion among employees of the responsibilities with the application of lockout on the Glue and Roll Coat Lines Oxidizer 1097. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
