29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) C 52" Coater Machine (5 Roll station): On or about July 1, 2018, the employer did not insure printing press machine specific procedures were utilized by employee(s) when cleaning condensation off of roller, in-that the roller was not deenergized and rotating. This exposed the employee(s) to ingoing nip point, crushing hazard and amputation injury. In the alternative... 1910.212(a)(1): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards. (a) Coater 52 Machine (5 Roll station):The employer did not insure printing press interlock was properly implemented as part of the integrated safety system, in-that the interlock was bypassed. This allowed the employee(s) to access the sealant roller in an energized state and exposed the employee(s) to ingoing nip point, crushing hazard and amputation injury.
