29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about July 5, 2018, and times prior, at a jobsite located at 6361 Singleton Boulevard, Dallas, Texas, employees were exposed to a fall hazard approximately 10 feet in depth by not protecting employees from falling into open manholes when working replacing sewer lines.
