29 CFR 1926.702(j)(1): Employees were permitted to perform maintenance or repair activity on equipment (such as compressors, mixers, screens, or pumps used for concrete and masonry construction activities) where the inadvertent operation of the equipment could occur and cause injury: At the time of the amputation injury on 6-25-2018, the Bidwell Roller Paver was not locked and tagged out. An employee was inside the operating area of the machine when a second employee's actions caused a spring loaded rod to release causing the amputation injury.
