29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: Worksite located 2500 Mission Drive, Jefferson City, Missouri- On or about July 2, 2018, two employees were injured when one employee attempting to install a piece of form wall fell on to the employee working beneath the elevated employee. Employees piecing together formwork are exposed to fall hazards and are not protected from falling by the use of adequate means of fall protection.
