29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. Location: 77 Clarkson Ave. Brooklyn, NY. On or about 6/21/2018, a) An employee standing on the formwork installing horizontal supports was not protected from falling. The employee was injured when he fell approximately 9 feet to the level below. REPEAT-2 (SECOND REPEAT) Magellan Concrete Structures Corp. was previously cited for a first repeat violation of this occupational safety and health standard, its equivalent, or essentially similar standard, 1926.501(b)(1), which was contained in OSHA inspection number 1187756, citation number 2, item number 1 and was affirmed as a final order on 8/21/2017 with respect to a workplace located at 321 Wythe Avenue Brooklyn, NY 11249.
