29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about June 18, 2018, at the freezer storage, an employee was exposed to struck-by hazards when his powered industrial truck (PIT #1) was stuck in a pot hole and a second PIT (#2) was attempting to remove his PIT out of the hole; the worker was impacted and injured by the forks of PIT #2 during this process when PIT #1 fell off the forks of PIT #2.
