29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Roofs Over Ice Machine Building and Engine Room #3; On or about June 12, 2018, and at times prior, the employer exposed employees to a 30 feet fall hazard, when employees jumped from one rooftop to another. b) Condenser Platforms; On or about June 12, 2018, and at times prior, the employer exposed employees to between 15 and 30 feet fall hazards when employees cleaned the condenser located on the roof of Engine Room #3
