29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) ContractOne, Inc. at 105 Base Camp Circle, Granby, CO 80446: On or before June 14, 2018, the employer did not ensure that each employee on the jobsite was trained on safety and health hazards in trenching operations. Two employees were installing water lines in trenches of approximately 8 feet deep without training regarding the hazards associated with this work including but not limited to water accumulation, trench wall fissures, sloughing of soil, cave-ins, and spoil pile hazards. This condition exposed employees to cave-in and struck-by hazards.
