29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: (a) On or about June 12, 2018 and times prior thereto, employees working inside of the trench were working in a 8-feet deep trench excavation where water was accumulating, were not protected from electrical hazards.
