(a) 130 Pike Lane - An unqualified employee was exposed to electrical shock hazards when assigned to line clearance tree trimming duties within 10 feet of an energized 34.5 Kv power line, the employee did not have the requisite training to recognize safety related work practices and procedures, on or about June 11, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
