29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. a) Employees were exposed to 120/208V energized parts and arc flash hazards when working on electric equipment that had not been de-energized; electric panel MP 14 had a arc-flash explosion. --- Employees did not wear arc flash PPE (personal protective equipment); arc flash suit, hood, face shield, gloves, rubber mats, barricades; Location: 200 E 59th Street, New York, N.Y. On or about 6/9/2018 Top Shelf Electric Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 1926.28(a), which was contained in OSHA inspection number1141807, citation number 01, item number 01 and was affirmed as a final order on August 17, 2017, with respect to a workplace located at 203 East 92nd Street, New York, NY.
