29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Worksite at the Waits 2-3-4-H 1709 wellsite. Kingfisher County, OK. On or about June 04, 2018, and at times prior thereto, adequate guarding was not in place on equipment used in the sand delivery system. Employees were exposed to caught-in hazards from the unguarded hydraulic ram(s).
