29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were independent of any anchorage being used to support or suspend platforms but were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: a. On May 31, 2018, on the 6:12 pitch roof, the fall protection anchor was installed in weakened plywood, causing it to be ripped loose from the plywood when the employee fell, exposing the employee to a 19 feet fall hazard.
