29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a.) On or about May 31, 2018, the employer did not ensure periodic inspections were completed for employees who performed lock out tag out activities. Employees were exposed to crush-by hazards while performing cleaning activities on equipment, such as but not limited to the P8 Paint Strip machine.
