On 5/23/18, at the Flowery Branch facility, the employer did not conduct a periodic inspection of the Lockout program to ensure deficiencies are corrected and verify all equipment had specific written procedures (Single Block B1603 was added to production 1 year 9 months earlier and Single Block D1602 was added 1 year ago).
