29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) On or about May 15, 2018, and at times prior, the employer did not report a work-related incident resulting in in-patient hospitalization, amputation, or the loss of an eye within 24 hours if the incident occurring. On May 9, 2018, an employee was hospitalized following a work-related incident. The employer, Zaremba Enterprises, did not report the incident until May 15, 2018.
