29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) West Laurel Interchange: The employer failed to report an in-patient hospitalization to OSHA within the mandatory reporting requirement of 24 hours. On May 04, 2018 an employee was injured and hospitalized, and OSHA was notified of the in-patient hospitalization on May 23, 2018.
