OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that workers were exposed to struck-by and crushing hazards from unsecured equipment attachments: a) Hewlett Packard Boise Campus - Between Buildings 6 and 7: On or about May 23, 2018, and at times prior thereto, employees operated a John Deere 5525 tractor with a 553 loader using after-market fabricated metal bins to collect and dump grass clippings weighing approximately 200-300 lbs. The attachment elevated was not fully secured and pinned to prevent displacement. NOTE: Among others, feasible and acceptable means of abatement may include following equipment owner's manuals and taking steps to ensure attachments are properly secured with safety latches and pins which are fully in place before operating. This may include the development and implementation of a work rule, training, periodic checks and enforcement of the rule.
