29 CFR 1926.95(c): Personal protective equipment was not of safe design and construction for the work to be performed: On or about May 18, 2018, and at times prior thereto, employees were demolishing the second floor of the building under construction for complete removal of the floor without the self-retracting lifelines being positioned overhead, exposing employees to a fall hazard.
