Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.94(a)(4)(i): The construction, installation, inspection, and maintenance of exhaust systems shall conform to the principles and requirements set forth in American National Standard Fundamentals Governing the Design and Operation of Local Exhaust Systems, Z9.2-1960, and ANSI Z33.1-1961, which are incorporated by reference as specified in �� 1910.6. a) On or about 05/17/18, in the grinding department at TPI Arcade, located at 7888 Route 98 in Arcade, New York, the employer did not maintain, inspect or clean the duct work of the dust collection unit used to ventilate and filter exhaust from a Wheelabrator Blast Machine, to prevent the accumulation of 9,560 lbs. or 4.7 tons of steel shot particles and cause the duct pipe to collapse. Employees were exposed to fatal crushing injuries. b) On or about 05/17/18, in the grinding department at TPI Arcade, located at 7888 Route 98 in Arcade, New York, the employer did not install an appropriately designed local exhaust system that includes clean out ports to facilitate inspection and maintenance as required by ANSI Z9.2-1960.
