29 CFR 1904.32(a)(2): The employer did not create an annual summary of injuries and illnesses recorded on the OSHA 300 log: a) 300A - The OSHA 300A or equivalent was not created at the end of the calendar year to show an annual summary of 2017's injuries and illnesses, on or about May 15th, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
