29 CFR 1910.23(d)(7): Fixed ladders. The employer must ensure grab bars extend 42 inches (1.1 m) above the access level or landing platforms served by the ladder; a) Vertical Rack System; On or about May 16, 2018, and at times prior, the employer exposed employees to fall hazards of approximately 5 feet 1.5 inches from the next lower level, in that; the fixed ladder grab bars were not extended 42 inches past the vertical rack system platform.
