29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. The employer is failing to report amputation incidents to the Area Office within the required 24-hour time frame from the event. On our about April 25, 2018 at approximately 7:30 pm an employee amputated a finger tip while operating a band saw at 3701 Aviation Rd., Lincoln, NE 68524. The employer reported the amputation to the Omaha Area Office on May 1, 2018, more than 24-hours after the incident occurred.
