29 CFR 1910.119(f)(1)(i): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process: Or in its alternative: 29 CFR 1910.119(j)(2): The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment: a) On or about April 27, 2018, in the PE South Unit located in Longview, Texas, the employer failed to establish written operating procedures on how to determine if the radar level detector on the site glass, for the D-1B 350 psig ethylene make-up tank, was adequate and working properly when the system becomes blocked or clogged with accumulated condensed liquids.
