29 CFR 1910.145(c)(2)(ii): All employees did not understand that caution signs indicate a possible hazard against which proper precaution should be taken. a) Antero Ritchie Petroleum Pad, Pennsboro, WV: On or about April 30, 2018, while employees were trained in caution signs warning of pinch point hazards associated with pump maintenance, the employer did not ensure that employees fully understood the importance of the warnings when conducting maintenance activities on Q10 high pressure pumps.
