29 CFR 1910.106(h)(8)(i): "General." Maintenance and operating practices shall be in accordance with established procedures which will tend to control leakage and prevent the accidental escape of flammable liquids. Spills shall be cleaned up promptly. On or about April 28, 2018 and times prior to, at a wastewater disposal site located in Hennessey, Oklahoma, the employer does not inspect pipes, valves, and other equipment used at the site to ensure mechanical integrity, exposing employees to struck-by hazards.
