29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that were 4 feet (1.2 m) or more above a lower level. a) On or about 05/04/2018 at the Lake Dora Villas Condominium, Mount Dora, an employee was exposed to a 16.5 foot fall hazard, in that, fall protection was not used while walking on a 4 in 12 pitched roof where skylights were not guarded.
