29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. The employer did not develop and/or implement written lockout/tagout procedures for each type of equipment or machine located within the plant. Employees performing maintenance work on the equipment such as, but not limited to Davis-Standard Extruder; a Sano winding machine; zum-box; and a wrapper machine were exposed to electrocution and/or amputation hazards.
