29 CFR 1910.119(j)(4)(iv): The employer shall document each inspection and test that has been performed on process equipment. This violation was most recently observed on or about April 19, 2018, in the main fractionator at Valero Refinery located in Texas City, Texas when the employer did not implement the Mechanical Integrity Inspection Manual-Unit Inspection Plan (Section 3.15.5) to ensure past inspection reports and data were maintained in PCMS (Plant Condition Management Software) or properly filed in the equipment folders, or archived.
