29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift. 1008 Smede Hwy, Broussard, LA (Evangeline Plaza): On or about April 18, 2018, the employer did not ensure that the employee working from a bucket lift at a height of approximately 30 ft. wore adequate fall protection. The employee was engaged in changing the lights on the Evangeline Plaza Shopping Center sign in the parking lot.
