29 CFR 1910.30(a)(1): The employer did not provide training for each employee exposed to fall hazards and who used personal fall protection systems or is required to be trained as specified elsewhere in this subpart: a) On April 11, 2018, the employer did not ensure that an employee working from heights was adequately trained in the recognition and avoidance of fall hazards. The employee was exposed to a 13 foot fall hazard to the concrete below while working on shelter house #2.
