29 CFR 1910.219(f)(3): Sprocket wheels and chains, which were seven feet or less from the floor or platform, were not enclosed: a) On or about 4/12/2018; an employee cleaning the Hytrol bundle conveyor was exposed to a caught-in hazard in that the drive chain and sprockets of the drive motor box located below the conveyor at a height of approximately thirty-four and one-half inches were not fully enclosed. PACKAGING CORPORATION OF AMERICA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, (1910.219(F)(3), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 955547, CITATION NUMBER 02, ITEM NUMBER 004, AND WAS AFFIRMED AS A FINAL ORDER ON 05/15/2015, WITH RESPECT TO A WORKPLACE LOCATED AT 708 KILLIAN ROAD, AKRON, OH 44319.
