OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush-by hazards. On or about April 14, 2018 in the 50-ton Bay area located at 5311 1-20 Service Road, West Monroe, LA, while an employee performed work beneath the Tending End of the Suction Couch Roll (SCR): (1) the roll was a non-standard roll in that its center of weight (gravity) was offset while it was elevated, and (2) the edge of the wooden block supporting the foot of the SCR was placed neither flush with nor protruding beyond the edge of the SCRs foot but was two inches short, resulting to that extent in a lack of support of the SCR; both factors together causing the offset roll to be positioned so as not to prevent uncontrolled movement, creating a crushing hazard. Implement and retrain employees in the employers recently-updated (since the accident) Work Instruction W-I 704 Identifying Unsafe Rolls (Revision: 3) that addressed identification of rolls determined by management to have a safety hazard and require special care to lift, place or handle, or have the potential for tipping or other hazards, as well as to establish methods used to notify all persons handling rolls of unsafe conditions until remedied.
