29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces are permit-required confined spaces: In the Boiler Room, Hopper: On and before April 08, 2018, the employer did not evaluate the Veto 6 Fuel Burner bin (hopper) as a permit-required confined space at the workplace. Employees were required to enter a fuel burner bin (hopper) approximately 2-3 times a week to remove large wood chunks from the conveyor and hopper.
