29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: Rig # 7. Worksite at 1-30 Wewoka, Seminole County, OK. On or about April 09, 2018, employees were exposed to a fall hazard of 4 feet of more in the following instances: a) Employee working from operators console at a height of 55 inches without the use of guardrail system or other forms of fall protection. b) Employee working from rod basket at a height of 75 feet with a guardrail system that lacked a midrail. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation describing the actions it is taking to establish, communicate, and implement policies for the fall protection use for employees exposed to falls at a height of more than 4 feet.
