29 CFR 1926.21(b)(2): The employer did not ensure all employees were instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about April 05, 2018 and at times prior thereto, at the Trail Ridge Drive Trenching/Utilities Project, the employer did not ensure all employees working in and around an excavation were provided excavation and trenching safety training through the employer's training program, potentially exposing certain employees to a struck by and engulfment hazard.
