29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each powered industrial truck operator had successfully completed training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: a) Tannersville, PA: Powered industrial truck operators were operating equipment such as the Mitsubishi electric forklift model #430, did not receive training consisting of formal instruction and an evaluation of the operator's performance, on or about April 6, 2018. Neovia Logistics, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1254755, citation number 1item number 1 and was affirmed as a final order on 12/5/2017 , with respect to a workplace located at 3930 Cedar Creek Drive, Joliet, IL 60436.
